AUTHORIZATION FOR MEMBERSHIP DUES WITHHOLDING, AUTHORIZATION FOR STATE COPE, ACTIVATE $5,000 OF GROUP LIFE INSURANCE AT NO COST TO YOU
By providing the information on this form, I authorize Las Cruces Transportation Federation - Local 06341 and the American Federation of Teachers (AFT) to deduct dues for membership per the above information and in accordance with the Las Cruces Transportation Federation - Local 06341 constitution, the state constitution and the AFT constitution. Dues payments are not deductible as charitable contributions for federal income tax purposes, but a portion thereof may be deductible as a miscellaneous itemized deduction. The dues amount may change if authorized according to the requirements of the local, state or national constitutions. If this happens, I authorize my bank or credit card to adjust my payment when notified by Las Cruces Transportation Federation - Local 06341. I agree this authorization remains in effect until terminated in writing by me or until Las Cruces Transportation Federation - Local 06341 is given written notification of my separation from my employer and/or the bargaining unit. As per Article 6 Section 4 of the Las Cruces Transportation Federation, Local 6341 Collective Bargaining Agreement: New employees hired after August 1, 2022 will have thirty-one (31) days following their date of hire to become and remain members in good standing in the Union. Employees may satisfy their obligation under this Article to become or remain a member of the Union by tendering to the Union that portion of periodic dues and/or fees universally required as a condition of acquiring or retaining full membership in the Union which is expended on activities which are necessary to performing the Union’s duties as an exclusive bargaining representative. (a) Upon failure of any employee to tender his/her dues and/or fees to the Union within the period, and under the conditions specified above, the Union shall notify the Employer and the employee in writing of such notice, and not more than seven (7) days thereafter, the Employer shall discharge such employee, unless the Union has notified the Employer that the employee has satisfied the requirements for good standing.

COPE DISCLOSURE: I hereby authorize a monthly contribution to the COPE in the amount indicated above. This authorization is signed freely and voluntarily and not out of any fear of reprisal, and I will not be favored nor disadvantaged because I exercise this right. I understand this money will be used to make political contributions. COPE may engage in joint fundraising efforts with AFT COPE and/or the AFL‐CIO. This voluntary authorization may be revoked at any time by notifying COPE in writing of the desire to do so. Contribution or gifts to COPE are not deductible as charitable contributions for federal income tax purposes. Contributions cannot be reimbursed or otherwise paid by any other person or entity.
