I hereby request and accept membership in the Faculty & Staff Federation of Community College of Philadelphia, AFT Local 2026, AFL-CIO, and authorize and designate it to be my representative for the purpose of collective bargaining with Community College of Philadelphia on matters of compensation, hours and all other conditions of employment.
I hereby authorize and direct Community College of Philadelphia to deduct from my earnings the amount necessary to pay union dues to the Faculty & Staff Federation of Community College of Philadelphia in a manner as set forth in the official notification filed by the Federation with Community College of Philadelphia. The amount so deducted shall be transmitted to the Faculty & Staff Federation of Community College of Philadelphia. This authorization may be revoked by me in accordance with the maintenance of membership provisions of the Collective Bargaining Agreement between the Faculty & Staff Federation of Community College of Philadelphia and Community College of Philadelphia by sending notice of such revocation by certified mail to both the Federation and Community College of Philadelphia.
