I authorize the Detroit Association of Educational Office Employees (DAEOE) to deduct monthly dues for membership in accordance with the DAEOE constitution and the AFT constitution.

By providing the information on this form, I authorize the Detroit Association of Educational Office Employees, to deduct bi-weekly dues for membership per the above information and in accordance with the DAEOE constitution and the AFT constitution. Dues payments are not deductible as charitable contributions for federal income tax purposes, but a portion thereof may be deductible as a miscellaneous itemized deduction. The bi-weekly dues amount may change if authorized according to the requirements of the local, state or national constitutions. If this happens, I authorize my bank or credit card to adjust my bi-weekly payment when notified by DAEOE. These deductions shall be made bi-weekly per the above information and continue until DAEOE is given written notification of cancellation or termination of employment with your school district.
